Corriere Dello Sport claim the Blues are ready to pay £13.7m for the 27-year-old as they look to overhaul their squad in the summer.
Inter star Perisic has been in fine form for Roberto Mancini this season scoring six goals and providing three assists in the Italian top flight.
Blues also want Brozovic but Arsenal have tracked the Croatian for months with the player admitting he will decide on his future at the end of the current campaign.
“I have heard about these rumours. For now, I am an Inter player and I will behave as such,” he told Croatian newspaper Jutarnji.
“Everything is possible. We’ll see. We’ll know more after the Euros.
“There’s nothing more to say at this time, it’s too soon.”
Brozovic – a central midfielder who can play in an attacking or defensive role – has netted six times and provided four assists in his 27 appearances this season.
